Kittitas County Health Network

Kittitas County is a rural county in Central Washington State. The service area and target populations have many barriers that prohibit access to health care. The Blewett, Manastash, and Snoqualmie Mountain passes border Kittitas County, which pose geographic barriers to accessing health care.

Erie County

The grant project involves a partnership among four entities delivering services across a rural, four-county area of northern Ohio. The consortium includes the Erie County Health Department/Erie County Community Health Center, The Road to Hope, The LCADA Way, and Genesis by the Lake.

Community Care of West Virginia, Inc.

This project is addressing treatment prevention and long term recovery in North Central West Virginia. Specifically, the counties of Lewis, Braxton, Upshur and Southern Harrison.

Chautauqua County

The strategic plan for this project seeks to address current gaps in services/access to care within Chautauqua County as identified through the consortium’ s assessment process, including:
• the need for even wider distribution of naloxone to prevent even more opioid overdose deaths;

The East Tennessee Appalachian region is noted for its vulnerability to opioid use disorder (OUD); its rural communities are particularly vulnerable, as emphasized by extreme rates of NAS and opioid overdose, and lack of treatment and recovery providers and services.

Our approach for this implementation project is based on the comprehensive community analysis and strategic planning completed during the RCORP-Planning phase. Each consortium member brings individual strengths and resources that complement the other members.

The goal of the project and consortium team is to reduce the morbidity and mortality of Substance Use Disorder (SUD), including Opioid Use Disorder (OUD), in our high-risk, rural community through our consortium and community partners by implementing prevention, treatment, and recovery service
