Indiana Rural Health Association

The grant project involves a partnership among four entities delivering services across a rural, four-county area of northern Ohio. The consortium includes the Erie County Health Department/Erie County Community Health Center, The Road to Hope, The LCADA Way, and Genesis by the Lake.
